WHEN THE PARENTS ARE MARRIED
  • 1. Medical birth certificate, drawn up on a standard form, which must bear the registration number, certain date, the seal of the health unit, the doctor's signature and initials (obtained on site at the maternity ward);
  • 2. The marriage certificate of the child's parents (original and copy);
  • 3. Identity documents regarding the parents, within the validity period, (bulletin/identity card/provisional identity card). If the birth declaration is not made by the mother or father, the identity document of the declarant is required. (original and copy);
  • 4. Written statement, it is requested if the parents are married but have a different surname, signed by both parents, in front of the civil status officer who registers the birth or the public notary, regarding the surname that the child will acquire;
  • 5. Declaration regarding the child's domicile, it is required if the parents have different domiciles, signed by the parent who declares the birth, in front of the civil status officer who registers the birth.
WHEN THE PARENTS ARE NOT MARRIED
  • 1. Medical birth certificate, drawn up on a standard form, which must bear the registration number, certain date, the seal of the health unit, the doctor's signature and initials (obtained on site from the maternity ward);
  • 2. Mother's birth certificate; (original and copy)
  • 3. Father's birth certificate; (original and copy)
  • 4. The mother's identity document, within the validity period, (bulletin/ identity card/ provisional identity card) (original and copy)
  • 5. Father's ID, within the validity period, (the bulletin/identity card/provisional identity card), if he recognizes the child; (original and copy)
  • 6. Declaration of recognition of the child, is attached if the parents are not married and the father of the child recognizes him, the declaration is given exclusively in front of the civil status officer who registers the birth, from which the surname acquired by the child can be derived, to which the mother's consent is attached, also given exclusively in front of the civil status officer who registers the birth.
WHEN THE PARENTS ARE FOREIGN CITIZENS
  • 1. Medical birth certificate, drawn up on a standard form, which must bear the registration number, certain date, the seal of the health unit, the doctor's signature and initials (obtained from the maternity ward); valid identity document (identity card/residence card for family members/residence permit/certificate of registration of the citizen of the EU or the European Economic Area or the Swiss Confederation - present together, as the case may be) of both parents;
  • 2. The passport of the foreign citizen parent/ foreign citizen or stateless parents;
  • 3. The marriage certificate of the child's parents (if appropriate) released by the authorities from:
    • – Romania,
    • – abroad (simple/ with apostille/ superlegalized) translated and legalized
  • 4. Statement given before the registrar, with the help of an authorized translator, made by both parents - to recognize paternity and bear the name - (when the child's parents, citizen/foreign citizens are not married).
